These documents are best filled out on a smartphone. If a smartphone is not available, complete them on a computer. Jericho Road Employees must fill in all of the yellow highlighted portions (except the "Signature of Employer or Authorized Representative" on pg. 2 of the I-9). When all pages are filled out, DO NOT click on the "submit" button. When pages are completed, click on the "DONE" button in the top right corner. After you have completed, the next page may have a request to upload documents (Social Security Card, Driver's License, Direct Deposit info, etc.). You may choose to upload or not upload.

Please complete all paperwork ASAP as our payroll company needs these documents as per requested.
